父母孕期吸烟和戒烟对儿童哮喘的风险

   背景:评估孕期母亲和父亲吸烟与子女哮喘之间的关系
   方法:我们在芬兰库奥皮奥大学医院妇产科进行了一项基于医院出生的回顾性观察性出生队列研究。39 306个妇女,在1989年到2006年之间分娩,与国家对他们的子女哮喘报销登记(2641名哮喘患者)联系起来。怀孕期间有妊娠因素的记录。
   结果:如果父母双方都吸烟,哮喘的风险显著升高(aOR 3.7;95%CI 3.2-4.4),只有父亲吸烟的家庭(aOR 2.9;95%CI 2.5-3.3)以及只有母亲吸烟的家庭(aOR 1.7;95%CI 1.2-2.2),哮喘的风险仍然很高。在怀孕期间,吸烟的父亲戒烟似乎可降低哮喘的风险(aOR 0.3-0.4),不管母亲是否吸烟。
   结论:父母吸烟,尤其是父亲吸烟,与子女患哮喘显著相关,吸烟的父亲在孕期戒烟与降低儿童哮喘风险相关,无论母亲是否吸烟。结果表明,应该鼓励父母双亲在怀孕期间戒烟,因为它是一个相对容易和廉价的方式来减少子女患上哮喘的风险。

Parental smoking and cessation during pregnancy and the risk of childhood asthma.

Abstract
BACKGROUND:To evaluate the association between maternal and paternal smoking during pregnancy, and asthma among offspring.
METHODS:We conducted a hospital-based birth retrospective observational birth cohort study in a University-based Obstetrics and Gynecology Department, Kuopio University Hospital, Finland. 39 306 women, delivering between 1989 and 2006, were linked to the national register for asthmareimbursement for their offspring (2641 asthmatics). Pregnancy factors were recorded during pregnancy.
RESULTS:The risk of asthma was significantly elevated if both parents smoked (aOR 3.7; 95 % Cl 3.2-4.4) and it remained high in only paternal smoking families (aOR 2.9; 95 % Cl 2.5-3.3) as well as only maternal smoking families (aOR 1.7; 95 % Cl 1.2-2.2). Paternal cessation of smoking during pregnancy seemed to reduce the risk of asthma regardless of maternal smoking (aOR 0.3-0.4).
CONCLUSIONS:Parental smoking, and especially paternal smoking, was significantly associated with the risk of asthma in offspring and paternal cessation of smoking during pregnancy was associated with a decreased risk of childhood asthma regardless of maternal smoking. The results indicate that both parents should be encouraged to quit smoking during pregnancy, since it is a relatively easy and cheap way to reduce the risk ofasthma in offspring.
