成人哮喘呼出气生物标志物

   摘要 寻找可以指导哮喘精准治疗的生物标志物,特别是可以转化为临床应用的生物标志物越来越受到关注。呼吸测试包括挥发性有机化合物(VOCs)、呼出气一氧化氮(FeNO)、呼出气体冷凝液(EBC)中物质的检测。本文就上述生物标志物的特性、检测方式及在成人哮喘临床中的应用进行阐述。

   关键词 哮喘;呼出气;标志物

   哮喘是一种异质性疾病,常以慢性气道炎症为特征。哮喘的诊断基于存在特征性症状的病史及可变的气流受限的证据,目前治疗以吸入支气管扩张剂及糖皮质激素为主1。而生物标志物,采用美国国立卫生研究院(NIH)的定义为:作为正常生物过程、致病过程或对干预的药理学反应的指标,是可以被客观测量和评估的特征2

   目前用于哮喘诊疗临床上可获得的生物标志物主要来源于支气管肺泡灌洗液、支气管组织活检、诱导痰、外周血等,因其复杂性、有创性、价格昂贵等特点应用受限3。呼出气相关检查因其无创、便捷、可重复性、易于接受的特点越来越受到关注,呼出气生物标志物的发现对于成人哮喘的诊断及监测提供了新的思路。本文将对VOCs、FeNO、EBC在成人哮喘中的应用作一简要阐述。

   1VOCs

   人体呼出气中包含数千种挥发性有机化合物即VOCs(volatile organic compounds,VOCs),它们能够反映机体内全身及气道局部的代谢过程,其便捷、无创、可重复性等特点使VOC相关研究在生物标志物探索中具有很好的前景。

   挥发性有机化合物存在于呼出气、尿液、血液、唾液和粪便中,也可由皮肤排出4。一方面VOCs源于内源性代谢,可能与之前的外源性暴露(吸烟、药物、食物等)有关或无关5,也可能源于肠道或气道中的细菌。另一方面呼出的VOCs还包括从环境中吸入的任何VOCs,包括采样装置。氧化应激(Oxidativestress, OS)参与了许多系统性和器官特异性疾病的发生和发展,包括心血管、肺、自身免疫性、神经性、炎性疾病和癌症6。OS在VOCs生物标志物的产生中起着重要的作用。

   总的来说,VOCs分析有两种方法。一种基于质谱的方法旨在检测单个挥发性有机化合物,并在某些情况下对其进行鉴别,识别与特定疾病状况和伴随的病理生理过程相关的生物标志物;另一种电子鼻是基于交叉反应的非特异性传感器阵列,识别VOCs混合物的模式,产生基于模式识别的定量信号变化7。目前用于分析挥发性有机化合物的仪器包括气相色谱-质谱分析(GC-MS)8,质子转移反应质谱(PTR-MS)9,选择离子流管质谱(SIFT-MS)10,气体传感器阵列(电子鼻)11,离子迁移谱(IMS)12和电喷雾萃取电离质谱(EESI-MS)13,但需要通过适当的校准程序进行质量控制。

   采样方法的选择取决于应用。与呼出气采样有关的因素包括:校正周围环境中的挥发物,采样类型(混合呼气与肺泡气),采样持续时间(单次呼吸与固定时间或固定呼吸量),呼气流量和屏气的影响,收集材料的类型,VOCs恢复,样品预处理,湿度,食物/药物,运动,吸烟和合并症等14

   当前哮喘相关VOCs研究着眼于哮喘诊断、表型分型、治疗分层、治疗监测和病情恶化预测/评估15。研究表明,VOCs信号能区分哮喘患者和健康受试者16及慢性阻塞性肺疾病(chronicobstructive pulmonary disease,COPD)患者17。通过纵向监测VOCs能够识别不同控制状态的哮喘18。一个更广泛的GC-MS研究19表明VOCs能够区分嗜酸性(3%截断值)与寡粒细胞性、中性粒细胞性哮喘(76%截断值),AUROC分别为0.99和0.88,并在一个复制队列中测试其模型,进行了验证;他们发现VOCs的特异性和敏感性优于FeNO和血嗜酸性粒细胞计数;壬醛、1-丙醇和己烷能够识别中性粒细胞哮喘,AUROC为0.73。然而,由于不同的呼吸收集策略和统计分析方法,无法从这些研究中可靠地比较电子鼻和GCMS。一项基于U-BIOPRED队列的研究20表明,呼出的VOCs与尿液中沙丁胺醇和口服皮质类固醇水平(通过液相色谱-高分辨质谱LC-MS检测)相关,证实了一些呼出的VOCs与哮喘药物有关,有希望发展成为监测病人体内药物水平变化的工具。

   但当前相关研究缺乏标准化的操作流程,缺乏外部验证,这限制了数据间的可比性。有必要进行大规模的多中心研究,比较不同的采样方法、设备和测定挥发性物质的方法,以解决标准化问题,并评估现有技术的当前临床应用。

   2FeNO

   呼出气一氧化氮(fractionalexhaled nitric oxide, FeNO)是近年来发展起来的一项新的无创气道炎症检测技术,具有定量、安全、便捷及可重复测定的优点,研究证实,FeNO与嗜酸粒细胞性气道炎症密切相关,从而广泛应用于哮喘的病情评估21

   NO存在于所有哺乳动物的器官系统中,可由人体肺组织产生。NO在肺/气道中的功能和作用反映了其作为血管扩张剂,支气管扩张剂,神经递质和炎症介质的关键作用22。哮喘患者呼出气中NO水平高,其气道上皮细胞中存在诱导型一氧化氮合酶(NOS2)的高表达23,NOS2表达上调常被认为是慢性气道炎症患者FeNO上升的主要原因24

   美国胸科学会/欧洲呼吸病学会(ATS/ERS)2005年发表了FeNO检测的标准规程25,指南建议,FeNO测量的标准呼气流速为50ml/s。影响FeNO测量结果的因素有年龄、性别、测量的装置和方法、吸烟、饮食、过敏体质、昼夜节律等。2011年美国胸科协会(ATS)发布指南26,再次肯定了FeNO作为气道炎性标志物的价值。目前FeNO已在临床上用于嗜酸粒细胞性气道炎症的诊断及慢性气道炎症性疾病激素治疗反应性的评估。美国胸科协会对FeNO的临界值定义为三级, 即低 (儿童<20ppb,成人<25 ppb) 、中 (儿童20~35ppb,成人25~50 ppb) 、高 (儿童>35ppb,成人>50 ppb) 。FeNO<25ppb提示患者为非嗜酸粒细胞性炎症且不建议使用激素治疗,FeNO>50ppb提示患者很可能存在嗜酸粒细胞性炎症, 并对激素治疗反应性好。

   研究发现,在长期咳嗽的受试者中,FeNO>38.8ppb时区分嗜酸粒细胞增多型哮喘与其他原因引起的慢性咳嗽,灵敏度为79.2%、特异度为91.3%27。FeNO在哮喘中增加,当哮喘控制状态下降或病情加重时,FeNO水平进一步升高28。在重症哮喘的研究29中,高FeNO的哮喘患者有更高的气道反应性、痰嗜酸性粒细胞、特应性和过度通气,但对症状的感知较差;高FeNO可识别出严重哮喘患者,其特征是最重的气流阻塞和过度通气,以及最频繁需要紧急护理,这表明FeNO为哮喘的严重程度提供了一个自主的分类,在重症哮喘患者中可识别最令人担忧的哮喘表型。另外,对成人过敏性重症哮喘和高FeNO患者的研究显示,在奥马珠单抗治疗后FeNO减少30,31,提示其在抗IgE治疗中作为疗效预测指标的作用。

   FeNO是一种无创的、易获取的生物标志物,对于预测哮喘的发展和恶化、识别特定的哮喘表型、改善特定人群的哮喘诊断和管理、以及监测糖皮质激素疗效具有重要意义,是用于哮喘诊断和治疗的一种有前途的监测手段。

   3EBC

   呼出气冷凝液(exhaled breathcondensate,EBC)是通过与冷凝器接触以冷却呼出气获得的。在EBC收集过程中,呼出气直接通过一个冷却装置,根据冷凝器的温度,导致呼出气成分的液相或固相积累。其收集过程是非侵入性的,只需潮气呼吸即可。收集不会改变气道表面状况,并且即使在年幼的儿童和患有严重肺病的成年人中也不会造成不良影响32

   EBC中绝大多数为水蒸气,还包含来自于气道内皮衬液的颗粒或液滴,以及水溶性的可挥发成分33。研究证实,EBC是一种有效的无创检测氧化应激相关生物标志物的方法34。研究发现哮喘患者EBC pH值低于健康对照组35,且控制不佳的患者EBC pH值低于得到控制的哮喘患者36,哮喘急性加重与气道pH值降低有关37。哮喘患者EBC中H2O2升高,Teng等人的荟萃分析表明38,EBC中H2O2与哮喘严重程度和控制水平相关。另外,EBC 中8-异前列腺素39、丙二醛与谷胱甘肽40等物质的浓度已被证明与潜在的哮喘及其严重程度有关,并可通过治疗干预进行调节。许多细胞因子、趋化因子和生长因子,如IL - 4、IL -8、IL -17,TNF-α,IFN-γ诱导蛋白10,TGF-β,巨噬细胞来源的趋化因子(MDC) ,嗜酸性粒细胞趋化因子和巨噬细胞炎性蛋白-1α及1β,在哮喘气道中明显上调41。

   利用高分辨率核磁共振光谱或质谱技术对EBC进行代谢组学分析,可以对内源性代谢产物进行特定的定量描述,从而提供一个可用于分类的代谢“指纹”。相关研究已证实EBC代谢组学分析可以区分健康受试者和不同炎症表型42及不同严重程度43的哮喘。EBC蛋白组学分析是研究气道疾病的另一种具有挑战性的方法,将蛋白芯片应用于EBC可同时检测许多潜在的生物标志物,为哮喘患者的EBC蛋白组学分析提供了一个很好的工具44

   4、总结

   当前利用代谢组学技术在呼出气中识别分子指纹印迹是可行的45,即所谓的呼吸组学(breathomics)。呼出气生物标志物形成了快速发展的研究领域,在哮喘诊断,炎症表型识别,治疗监测等方面显示出良好的优势。但这仍然是一个相对较新的领域,对其结果的解读仍需谨慎对待,并需要更规范化的实验和统计方法及外部验证,以更好地用于哮喘生物标志物研究。
